Opt Out Form
OSD Parents and Guardians,
Attached in this communication is the OSD opt-out form for Directory Information. As a reminder, Directory Information is a limited set of personal information about a student that is usually not considered harmful or an invasion of privacy. Examples can be programs for sports and information provided to colleges and technical schools. This can include student photos for the newspaper or school yearbook. We are required by law to provide parents with the option to opt-out of these publications.
If you would like to opt-out from Directory Information, please fill out and return the form to Mandy Lewis at the OHS high school office.
